By checking the consent box at checkout, you agree to the following:
Emergencies
This service is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. If you are in crisis or thinking about harming yourself, call or text 988 to reach the 988 Suicide & Crisis Lifeline. Do not use checkout or account messaging for urgent situations; providers do not monitor them in real time.
What you are consenting to
Your order includes products that require review and approval by a licensed healthcare provider before they can be fulfilled. That review happens asynchronously (sometimes called "store-and-forward" telehealth): you complete a health questionnaire, and a provider licensed in your state reviews your information and decides whether the requested products are appropriate for you. There is no video visit, phone call, or in-person examination unless the provider requests one. The reviewing provider may be a physician, a nurse practitioner, or a physician assistant licensed in your state.
You can choose in-person care instead at any time. Nothing here prevents you from seeing your own doctor, and you are encouraged to keep your regular healthcare provider informed of any treatment you receive.
How the review works
- You complete the health intake at checkout. The provider's decision is based on what you submit, so it must be accurate and complete.
- Our platform prepares the case for review, including an automated preliminary safety screen and a draft clinical note. Before any automated processing, your case is de-identified (your name is removed and your date of birth is converted to an age). Automated tools never decide your request; a licensed provider personally reviews and decides every case.
- The provider approves or declines the request, and may ask you for more information, a photo, or a live visit before deciding. You will be notified of the outcome, and the provider's decision is documented in a clinical note that becomes part of your medical record with the clinic. The provider decides the medication and dose, if any.
Benefits and risks
Benefits include convenient access to care without an appointment. Risks include those inherent to asynchronous care: the provider does not examine you in person, cannot observe things you do not report, and relies entirely on the accuracy of your answers, so a condition may be missed or a diagnosis delayed. Not every condition can be treated through telehealth, and the provider may decide you need an in-person examination, tests, or a specialist. No specific result is promised; your condition may not improve and could worsen. Delays in review, technical failures, or the need for additional information may delay your order. Electronic transmission and storage of health information carry a risk of interruption, unauthorized access, or loss, and ordinary email or text messages are not secure. Licensing and state law may limit or change the treatment available to you depending on where you are located.
Prescriptions, pharmacies, and payment
- A prescription is written only when the reviewing provider decides it is appropriate. Approved prescriptions are sent to a licensed U.S. pharmacy that ships to your state.
- Some products may be compounded medications. Compounded medications are prepared by a licensed pharmacy for an individual patient and are not reviewed or approved by the U.S. Food and Drug Administration for safety, effectiveness, or quality.
- You may ask to have an approved prescription filled by a pharmacy of your choice, or transferred between pharmacies, where the law and the pharmacy allow it; pricing may differ.
- Medications that cannot be prescribed through asynchronous telehealth under federal or state law will not be prescribed through this service.
- Care is self-pay. The clinic does not bill health insurance, Medicare, or Medicaid for these services and does not submit claims on your behalf.
Your rights
- Consent is voluntary. You may decline, and you may withdraw consent at any time before a decision is made, without penalty; withdrawal cancels provider review of the pending order.
- Approval is not guaranteed. The provider may decline any request; if so you will receive a refund per the store's refund policy.
- You have the right to receive a copy of your records; direct requests to the clinic named on this storefront.
- You may be entitled to follow-up care from the clinic; contact information is provided on the storefront and in your order communications.
- If you have a concern about the conduct of a licensed provider, you may contact the medical board or other professional licensing board of the state in which the provider is licensed.
Important limits
- This is not emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use this service for urgent conditions.
- A provider-patient relationship with the reviewing provider is formed only for the scope of this review.
- You attest that the information you provide is true, complete, and about you (not another person), and that you are at least 18 years old and located in the state you selected at the time of the request.
Who is involved
The clinic operating this storefront is the responsible healthcare provider. The review is performed by a provider licensed in your state. MDside AI LLC provides the technology platform and processes your information on the clinic's behalf under HIPAA; see the Privacy Policy.
Records of your consent (the document you were shown, its version, the time, and the network address it was given from) are retained with your order. You will be asked to accept the current version each time you place an order.
[Attorney: add state-specific riders where required, e.g. states mandating synchronous modalities for certain prescriptions, state consent language requirements, and any controlled-substance exclusions.]
